Code coverage with lcov

Includes:
 - A cmake script to setup lcov targets
 - A predefined cppast_coverage target (Disabled by default, enable by setting CPPAST_TEST_GCOV var to TRUE/ON/1)
 - Coverage report submission to coveralls.io
