- Restructure runtime code into declarations, function
  and class definitions, and initialisation code
- Rename internal functions/types to follow SWIG_Octave...
  or SwigOct... naming styles
- Style/comment/whitespace cleanups

4308dd03cf SWIG license change - The Examples and Lib move to a very permissive license in the LICENSE file, removing the BSD license restrictions as agreed by committers since it was inadvertently introduced. Remove some examples where the impact of the license change is not clear.

b6e44f2427 Octave: add {save,load}_{ascii,binary,hdf} functions that silently do nothing; 
give default operator mappings a more conventional suffix;
fix test li_std_string_extra.
