Olly Betts 9d7c962ee8 Clean up uses of Replace() 
We were passing flags of DOH_REPLACE_ANY|DOH_REPLACE_FIRST in three
places, which doesn't make sense as those are mutually exclusive
concepts.  In the current implementation DOH_REPLACE_FIRST wins in
this situation, so replace with that and clarify the docs.

SWIG Source directory

 Source/DOH          -  A core set of basic datatypes including
                        strings, lists, hashes, and files.  Used
                        extensively by the rest of SWIG.

 Source/Swig         -  Swig core. Type-system, utility functions.

 Source/Preprocessor -  SWIG C Preprocessor

 Source/CParse       -  SWIG C Parser (still messy)

 Source/Modules      -  Language modules.

 Source/Include      -  Include files.
